2026 ALLIANCE Hockey Annual Meeting Registration

For the purpose of registration, all attendees including Guests are referred to as "Delegates". If you need to register more than 12 people, please complete an additional form.

ALLIANCE Hockey has secured the most competitive fees from our Partners and has provided further subsidization of the fees below in an effort to maximize Annual Meeting Weekend participation. Options to submit payment are available at the bottom of the form.

Please read carefully before completing the registration:

- Registration deadline is May 31, 2026.
- Cancellations June 1-10, 2026 will be provided minus 25% of the specific fee.
- NO REFUNDS will be provided after June 11, 2026.

Includes: Green fee, lunch, driving range access, power cart, special gift, event competitions

BUS TO/FROM GOLF: $25.00

When: June 19, 2026, 9:30am Pick up at Hotel, Return from Golf Course following golf
Includes: Round trip motor coach transportation with golf club storage
