Certificate of Insurance Request Process
Hockey Canada’s insurance provider – "BFL Insurance" – has implemented a NEW online Certificate of Insurance Request procedure providing an easy step-by-step process with which to request a certificate of insurance for all Hockey Canada approved activities and events, such as Dryland Training, Fundraising, etc.

Please ensure that the Facility/Township/School Board’s Rental Agreements or contracts including the Terms & Conditions are downloaded in the area provided on the BLF site.

Meeting or other facility room for team or club meetings
PLEASE NOTE:
1. You must attach a copy of the rental agreement with this request.
2. Requests submitted less than two (2) weeks before rental may not be processed.

Events like year-end banquets or public relations, or club promotional events
PLEASE NOTE:
1. You must attach a copy of the rental agreement with this request.
2. Requests submitted less than two (2) weeks before rental may not be processed.
3. Not all fundraising and special event activities are permitted by the Ontario Hockey
