2024 ALLIANCE Hockey House League Championships - Payment
This form is to be completed by a Member Association Representative. Please indicate the number of team(s) your Association will send to the 2024 ALLLIANCE Hockey House League Championships and provide the accompanying payment no later than March 15, 2024. NOTE: Individual Teams will be asked to complete a Team Registration form and submit a roster and travel permit once those teams have been determined.
