HDCO Tom Pashby Trainer of the Year Award
The Hockey Development Centre for Ontario is once again accepting nominations for the 2014 Dr. Tom Pashby Trainer of the Year Award for the 2013-2014 season.

This award recognizes those individuals who demonstrate outstanding dedication and service in the areas of safety and risk management as well as a commitment to the values of safety, enjoyment, fair play, education and integrity in the game of hockey through the Hockey Trainers Certification Program in the province of Ontario.

Each association wishing to participate in this program can select a nominee within their respective jurisdiction based on the attached criteria.  Participating member associations are asked to complete the nominee form below and submit to the HDCO, where a selection committee will review and select a Provincial winner from the candidates submitted. 

Please note that each regional nominee will receive an award, which will be provided by the HDCO.  The Provincial winner will receive an additional award.   The award can be presented to the recipient at your respective AGM or at an appropriate ceremony of your choosing. 

Completed nomination forms are to be send to the HDCO office by April 1,2014.   Selection criteria and nomination form are below.

DR. TOM PASHBY TRAINER OF THE YEAR AWARD

 

SELECTION CRITERIA and NOMINATION FORM

          THE NOMINEE:

 

Ø  Must have attained Level III of the Hockey Trainers’ Certification Program or higher (HTCP Course Conductor or Master Course Conductor status).  *Equivalent training in the opinion of the HDCO member association will also be recognized.

 

Ø  Must be a member of an Ontario Hockey Canada Branch/Division

 

Ø  May either be in a volunteer or professional capacity

 

Ø  May be active or retired

 

Ø  May be nominated any number of years, however, can only receive the Provincial award once



1.
A completed nomination form must be submitted indicating outstanding achievements in the areas of Safety and Risk Management and/or contribution to the Hockey Trainers Certification Program.

 

2. The nominees will be forwarded to the HDCO selection committee, where one Provincial winner will be selected.

 

3. An award will be provided to each first time regional nominee submitted by an HDCO member association.  Previous regional recipients will not receive an award but will receive a letter of acknowledgement.

 

4. The Provincial recipient will receive an additional award, which can be presented at a function at the HDCO member’s discretion.
